Chronic Disease PATH Telephone Toolkit
Chronic Disease PATH (Personal Action Towards Health) is an evidence-based program designed for adults living with chronic or long-term health conditions including arthritis, heart disease, diabetes, emphysema, asthma, bronchitis, and depression. The 6-session telephone workshop meets for 1 hour, once a week for 6 weeks with a small group of 2-5 people. A variety of self-management tools such as relaxation techniques, placing and planning and weekly goal setting are explored to empower participants to take a more active role in managing their health condition.

Dates/Time: Wednesdays, March 20 - May 1, 2024 from 11:00am-12:00 pm, ET.
Cost: There is no cost for the workshop.
