Chronic Disease Prevention Coalition

Our Mission

The mission of the Northwest Michigan Chronic Disease Prevention Coalition is to organize and promote chronic disease-related services and prevention opportunities available in our service region of Crawford, Kalkaska, Lake, Manistee, Mason, Mecosta, Missaukee, Newaygo, Oceana, Osceola and Wexford counties. 

Our Vision

To improve the quality of life through an area wide organization by providing a collaborative delivery of services based upon community needs.

We strive to achieve our vision by doing the following:

  • Promote policy and environmental changes to reduce chronic disease

  • Communicate chronic disease impact

  • Educate constituent groups and the public about chronic disease

  • Collaborate with chronic disease-related organizations

  • Manage chronic disease-related resources

  • Serve as a resource to local coalitions in member counties

Tobacco Cessation Community/Clinical Linkages Task Force addresses the issue of high rates of tobacco use throughout our region
