Our Mission
The mission of the Northwest Michigan Chronic Disease Prevention Coalition is to organize and promote chronic disease-related services and prevention opportunities available in our service region of Crawford, Kalkaska, Lake, Manistee, Mason, Mecosta, Missaukee, Newaygo, Oceana, Osceola and Wexford counties.


Our Vision
What we hope to see accomplished in 3 to 5 years as a result of our actions:

Environmental change to increase physical activity and healthy eating

Increased prevention measures targeting specific chronic diseases

Improved access to both care and prevention

Improved engagement toward self-management through participation in evidence-based programming

Funding in place for mental health availability and pain/addiction center

Policy, Systems, and Environmental Change Task Force creates changes to reduce risk of chronic disease in our local population

Diabetes Prevention Program Task Force coordinates with providers to achieve the best outcome for our region on diabetes prevention
TOBACCO
TASK FORCE
Tobacco Cessation Community/Clinical Linkages Task Force addresses the issue of high rates of tobacco use throughout our region

Meetings
The NMCDPC meets quarterly, in December, March, June, and September, at the annual Summit. The location for the quarterly meeting rotates across the region. Call-in options are available for the meetings by request.
