Chronic Disease Prevention Coalition
The mission of the Northwest Michigan Chronic Disease Prevention Coalition is to organize and promote chronic disease-related services and prevention opportunities available in our service region of Crawford, Kalkaska, Lake, Manistee, Mason, Mecosta, Missaukee, Newaygo, Oceana, Osceola and Wexford counties.
To improve the quality of life through an area wide organization by providing a collaborative delivery of services based upon community needs.
We strive to achieve our vision by doing the following:
Promote policy and environmental changes to reduce chronic disease
Communicate chronic disease impact
Educate constituent groups and the public about chronic disease
Collaborate with chronic disease-related organizations
Manage chronic disease-related resources
Serve as a resource to local coalitions in member counties
TASK FORCE GROUPS
Policy, Systems, and Environmental Change Task Force creates changes to reduce risk of chronic disease in our local population
Diabetes Prevention Program Task Force coordinates with providers to achieve the best outcome for our region on diabetes prevention
Tobacco Cessation Community/Clinical Linkages Task Force addresses the issue of high rates of tobacco use throughout our region
