Taking care of your heart is essential for a long life. Heart disease is the number one killer of adults in the United States.
What is LiveWell for your Heart?
The Health Resources and Services Administration (HRSA) has provided grant funds to health partners in Lake and Mecosta County to help reduce the risk of heart disease in adults and increase access to health care services over the next 3 years.
Simple screenings will be done using the Heart Age Calculator for eligible adults to determine your risk of heart disease.
You can reduce your risk for heart disease.
Learn more about leading a healthier life.
Get connected to services such as;
Dental health care
Help to quit tobacco use
Physical activity resources
Diabetes prevention classes
Diabetes case management
High blood pressure management
What are the requirements?
You must be between the ages of 30 and 74 years old.
You have never been told you have heart disease.
Your live in or receive health services in Lake or Mecosta County.
Agree to complete the Heart Age calculator annually.
Agree to keep in touch with us for the next 3 years (1-3 calls a year).
