LiveWell For Your Heart
Taking care of your heart is essential for a long life. Heart disease is the number one killer of adults in the United States.
What is LiveWell for your Heart?
District Health Department #10, Baldwin Family Health Care, My Community Dental Center, Spectrum Health Big Rapids Hospital and Ferris State University are partnering in Lake and Mecosta County to help reduce the risk of heart disease in adults by increasing access to health care services and programs to reduce the risk of heart disease.
In Year 1, screenings were conducted with adults ages 30 – 74 using the CDC Heart Age Calculator. For more information about Heart Age click here
Between October 2018 and July 2019, 577 people in Lake and Mecosta county were screened and joined the Live Well for Your Heart Program.
Year 2 of the program consists of Live Well for Your Heart partners making calls to participants for re-screenings to see if their risk of heart disease has lowered.

Dental health care
Physical activity resources
Diabetes prevention classes
Diabetes case management
High blood pressure management
